FDA Approves Afrezza for Children: A New Needle-Free Mealtime Insulin Option

The U.S. Food and Drug Administration (FDA) has approved Afrezza®, an inhaled mealtime insulin, for children and adolescents ages 6 and older living with type 1 or type 2 diabetes. The approval makes Afrezza the first and only needle-free mealtime insulin available for pediatric patients in the United States.
What Is Afrezza?
Afrezza is a rapid-acting insulin that is inhaled through a small handheld device instead of injected with a syringe, pen, or insulin pump. Once inhaled, the insulin is absorbed through the lungs and enters the bloodstream quickly.
Unlike many injectable mealtime insulins that are taken before eating, Afrezza can be taken at the beginning of a meal, providing more flexibility when kids have unpredictable appetites or eating schedules.
Why This Matters
Managing diabetes in children is challenging. Kids may not finish the meal they planned to eat, decide they want seconds, or suddenly head outside to play sports. Those everyday situations can make insulin dosing stressful for both children and parents.

Who Can Use It?
The FDA approval includes children and adolescents ages 6 and older with:
• Type 1 diabetes (used together with a long-acting basal insulin)
• Type 2 diabetes when mealtime insulin is needed
Afrezza is not a replacement for long-acting insulin in people with type 1 diabetes. It is designed specifically for mealtime blood sugar management.
Is It Right for Everyone?
No. Because Afrezza is inhaled through the lungs, it is not recommended for everyone.
People with asthma, chronic obstructive pulmonary disease (COPD), or certain other lung conditions should not use Afrezza because it can cause serious breathing problems. Healthcare providers also perform a lung function test before starting treatment and periodically afterward.
Afrezza is also not intended for treating diabetic ketoacidosis (DKA).
What Families Should Know
The approval was supported by the Phase 3 INHALE-1 clinical trial, which demonstrated that Afrezza provided blood sugar control comparable to traditional mealtime injections while offering children and families a needle-free alternative. Clinical experts believe this additional option may help reduce some of the daily burden of diabetes management.
